Evaluation and Form Requests

Cherokee Health Systems (CHS) providers usually do not complete the following evaluations and forms:

  • Disability evaluations
  • Attorney-drafted forms related to disability
  • Conservatorship evaluations and forms
  • Payee forms
  • Child custody evaluations
  • DCS mental health or alcohol and drug evaluations
  • Emotional support animal documentation for air travel

If you are applying for disability, conservatorship, change in payee, or have another evaluation request that includes documentation of your healthcare information, you may request a release of records directly to you, your attorney, or a third party (e.g., Social Security).

If you do choose to release your records, please be mindful that your healthcare record includes detailed and sometimes sensitive information. It is your choice whether you wish for this information to be released to a third party.

What is Cherokee Health Systems?

Cherokee Health Systems (CHS) is a Federally Qualified Health Center that serves about 70,000 people annually. We offer a wide array of comprehensive health services, including primary care, behavioral health, dental, optometry, and pharmacy. All of our services are available to children, adults, and seniors on an ability to pay basis. CHS is a TennCare (Medicaid) and Medicare provider. In addition, CHS accepts commercial insurance for payment.

If you have never been to CHS before, you can call our Central Access Department toll-free at 1-866-231-4477 (between the hours of 8 a.m. - 5 p.m. EST).
